What Percent of Thyroid Biopsies are Cancerous?

Your healthcare provider may suggest you undergo a thyroid biopsy to determine whether a lump, nodule, or other abnormality in the thyroid is cancerous. Should you be concerned? What are the odds that the thyroid biopsy confirms you have cancer?

The good news is that the vast majority of thyroid abnormalities are benign – only about 5–15% of thyroid biopsies confirm cancer. Many thyroid growths are related to noncancerous conditions such as Hashimoto’s thyroiditis, multinodular goiter, or simple cysts. However, this doesn’t mean you should ignore thyroid issues – early diagnosis allows you to address any potential problems in their earliest and most treatable stages.  

5 Warning Signs That a Thyroid Nodule May Be Cancerous

In most cases, you won’t be recommended to get a biopsy unless your physician notices nodules. A thyroid nodule is an abnormal lump or growth within the thyroid gland. While most thyroid nodules are harmless, certain signs should prompt you to seek evaluation by a medical professional:

  1. Rapid Growth of the Nodule

Benign nodules tend to grow slowly, if at all. A nodule that enlarges quickly over weeks or months warrants closer investigation.

  1. Hard or Fixed Lump

Cancerous nodules are often firm or hard to the touch and may feel anchored to surrounding tissue, rather than moving freely under the skin.

  1. Persistent Hoarseness or Voice Changes

If a nodule presses on the recurrent laryngeal nerve, it can cause changes in voice quality. This symptom should be evaluated promptly.

  1. Difficulty Swallowing or Breathing

Larger nodules or tumors can press on the esophagus or trachea, causing a sensation of fullness, trouble swallowing, or shortness of breath.

  1. Swollen Lymph Nodes in the Neck

Enlarged or tender lymph nodes—especially if accompanied by a thyroid lump—may be a sign that cancer has spread beyond the thyroid.

Why a Thyroid Biopsy Might Not Be the Best Course of Action

A thyroid biopsy is a type of fine needle aspiration (FNA) biopsy. The procedure is generally safe, but like all medical interventions, there’s the possibility of complications. Potential negative effects include:

  • Bleeding and Bruising – Minor bleeding at the site is common, but for those with clotting disorders, the risk is higher.

  • Infection – While rare, infection around the injection site can occur.

  • Pain and Swelling – Temporary discomfort or inflammation can affect swallowing and neck mobility.

  • False Negatives or Indeterminate Results – A biopsy can miss cancer cells if the sample is too small or from the wrong area, leading to repeat procedures.

  • Scar Tissue Formation – Repeated biopsies can lead to scarring that may alter the thyroid’s appearance or function.

These risks highlight why a biopsy should be ordered only after a thorough evaluation and why alternative or complementary testing options may be worth considering.

Thyroid Conditions That Can Mimic Cancer

So, before you proceed with a biopsy, remember that many thyroid issues can cause nodules, swelling, or abnormal imaging results, without being cancerous. Some of the most common of these include:

  • Multinodular Goiter – An enlarged thyroid containing multiple benign nodules, often due to iodine deficiency or long-term hormonal imbalance.

  • Thyroid Cysts – Fluid-filled sacs that can look suspicious on imaging but are rarely cancerous.

  • Graves’ Disease – An autoimmune disorder causing hyperthyroidism and potential thyroid enlargement, sometimes with nodules.

  • Thyroid Adenomas – Noncancerous growths that can occasionally produce excess hormones, but don’t invade surrounding tissue.

Proper evaluation of these conditions often includes blood work, ultrasound imaging, and medical history review, sometimes avoiding unnecessary biopsies altogether.

New and Emerging Cancer Testing Methods

Finally, while biopsies remain the gold standard for diagnosis, newer and less invasive tests are becoming more widely available:

  • Blood-Based Cancer Tests – Some advanced blood tests can detect DNA or other molecular markers released by cancer cells, offering a noninvasive way to assess cancer risk.

  • Ultrasound Elastography – Measures tissue stiffness to help distinguish between benign and malignant nodules.

  • High-Resolution Ultrasound with AI Analysis – Artificial intelligence is being integrated to improve accuracy in identifying suspicious nodules.

  • Molecular Testing on FNA Samples – Molecular profiling can help predict the likelihood of malignancy and guide treatment.

These approaches can be combined to reduce unnecessary invasive procedures and improve diagnostic accuracy.
